Özet

Amaç: Bu çalışmada ortopedik ve travmatolojik cerrahi geçiren vakalarda uygulanan anestezi yöntemlerini ve bu yöntemlerin perioperatif mortalite ve morbiditeye etkisini retrospektif olarak araştırmayı amaçladık. Yöntem: Bu çalışmada 2006-2012 tarihleri arasında alınan ortopedi ve travmatolojik cerrahi geçiren vakaların yaş, cinsiyet, tanı, uygulanan anestezi yöntemi, ASA skoru, kan transfüzyonu yapılıp yapılmadığı, ameliyatın acil veya elektif olma durumu, ek hastalık varlığı, perioperatif görülen komplikasyonlar ve hastanın ameliyat sonrası yoğun bakım gereksinimi incelendi. Bulgular: Kliniğimizde 2006-2012 yılları arasında 2814 olgu ortopedik cerrahi geçirdi. Bu olguların %62,1’i erkek, %37,9’u kadın idi. Olguların %68,5’i genel, %31,5’si rejyonel anestezi ile opere oldu. Ameliyat sonrası olguların %97,4’ü postoperatif servise, %2,6’sı postoperatif yoğun bakım ünitesine (YBÜ) gönderildi. YBÜ'ye alınan olguların %23,6’sı ASA I, %14’ü ASA II, %62,5’i ASA III idi. Tüm olguların %10’unda en az bir komplikasyon görüldü. Genel anestezide komplikasyon görülme oranı %103, rejyonel anestezide %9,1 olarak tespit edildi. Tüm olguların %12,4’üne kan tranfüzyonu yapıldı. Olguların 182’sinde ek hastalık mevcut iken, 2632’sinde ek hastalık yoktu. Ek hastalık bulunan olguların %69’u ameliyat sonrası servise, %31’i YBÜ’ye gönderildi. Operasyon süresince en fazla görülen komplikasyon hipotansiyon, ikinci sırada bradikardi oldu. Sonuç: Ortopedi ve travmatoloji cerrahisinde rejyonal anestezi yöntemleri perioperatif komplikasyonları azaltabilir ancak olgunun karakteri seçilecek anestezi tekniğinde esas belirleyici olmalıdır. Yüksek riskli veya yandaş hastalığı bulunan hastaların acil ve kompleks cerrahilerinde kan transfüzyonu gereksinimi göz önüne alınarak anestezi planı yönetilmeli ve postoperatif yoğun bakım takibi muhakkak değerlendirilmelidir. 

Objective: In this study, the cases which were operated in our Orthopaedics and Traumatology clinic were retrospectively analyzed. However, we aimed to investigate retrospectively the effect of the anesthesia methods on perioperative mortality and morbidity. Method: In this study, age, gender, diagnosis, type of anesthesia, ASA clinical classification, intraoperative blood tranfusion requirement, type of surgery, emergency or elective, the presence of comorbid diseases, perioperative complications, postoperative transfer of the patients to the intensive care unit (ICU) requirement was analyzed in cases of orthopedics and traumatology surgery, between the dates o f2006-2012. Results: Between the dates of 2006-2012, 2814 patients had Orthopaedics and Traumatology surgery. Sixty two point one percent of these patients were male and 37,9% were female, 68,5% of the cases were operated with general anesthesia, 31,5% of the cases with regional anesthesia. After the surgery 97,4%o of the patients were transferred to service, 2,6%o were transferred to intensive care unit. The patients who received intensive care were 23,6%o of ASA I, 14%o ASA II, 62,5%o ASA III. In 10%o of all cases at least one complication was observed. The incidence of complications is, 10,3%o in general anesthesia, 9,1%o in regional anesthesia. In 12,4%o of all patients blood transfusion was performed. O f the all cases, 182 patients had additional diseases, whereas 2632 patients had no additional disease. Postoperatively, 69%o of patients with comorbid disease was transferred to service and 31%o of them were transferred to the ICU. The most common complication during the operation was hypotension, latter was bradycardia. Conclusion: In orthopaedic and traumatology surgery, regional anesthesia techiques may decrease the risk of perioperative complications but the medical risk factors of patients must be the basic predictor for the decision making of the anesthesia techniques. In the complex surgery of the patients with high ASA class or with accompanying diseases, anesthesia decisions should be made keeping in mind that there may be a need for blood transfusion. Postoperative follow up of the intensive care should definitely be evaluated as well.
