ÖZET
Amaç: Büyüme gelişim dönemindeki iskeletsel sınıf 3 anomaliye sahip bireylere zamanında müdahale etmek oldukça önemlidir. Bu olgu sunumunda, büyüme gelişim dönemindeki iskeletsel sınıf 3 anomaliye sahip bir bireyin hızlı üst çene ekspansiyonu ve yüz maskesi ile tedavisinin sonuçlarının değerlendirilmesi amaçlanmaktadır.
Materyal-Metot: Kronolojik yaşı 9 yıl 6 ay olan kadın hasta kliniğimize üst çenesinin geride olması şikayetiyle başvurmuştur. Hastanın ekstra oral muayenesinde maksiller sagittal yetmezlikle birlikte konkav profil tespit edilmiştir. İntraoral muayenesinde ise maksiller transversal yetersizlikle birlikte hastada dişsel Sınıf III ilişki, -3 mm overjet, 3 mm overbite tespit edilmiştir. Hastanın el-bilek filminin radyografik analiz sonucu mp3= döneminde olduğu belirlenmiştir. Maksiller retrognati (SNA=76°) ile karakterize iskeletsel sınıf III anomaliye (ANB= -3°) sahip hastaya bonded RME ve yüz maskesi apareyinin uygulanmasına karar verilmiştir. Hızlı ekspansiyon protololü sonucu vida günde iki tur çevrilmiştir. Sutur açıldıktan sonra yüz maskesi apareyinin kullanımına geçilmiştir. Elastikler tek tarafta 400 gr kuvveti okluzal düzlemle 30° açı oluşturacak şekilde ayarlanmıştır. Yüz maskesi apareyi 6 ay uygulandıktan sonra ön çapraz kapanış ile birlikte iskeletsel ve dişsel Sınıf III anomali düzeltilmiş ve yumuşak dokularda da başarılı değişiklikler elde edilmiştir.
Bulgular: Hastanın tedavi başlangıcında -3° olan ANB değeri, RME yüz maskesi kullanımı sonunda +3° olarak belirlendi.
Sonuç: Hasta tedavi sonunda kabul edilebilir estetik bir profile sahip oldu. Sınıf I molar ilişki ve ideal overjet, overbite ilişki elde edildi. Anahtar kelimeler: iskeletsel sınıf III anomali, hızlı üst çene genişletilmesi, yüz maskesi
ABSTRACT
Objective: It is critical to treat in individuals with skeletal class III anomalies during the growth and development period. The Objective of this case study is to evaluate the outcomes of treatment with rapid maxillary expansion and a face mask in an individual with skeletal class III anomalies during the growth and development period.
Material-Method: A female patient with a chronological age of 9 years and 6 months applied to our clinic with the complaint of lagging upper jaw. Extra-oral examination of the patient revealed a concave profile with maxillary sagittal insufficiency. During the intraoral examination, the patient with maxillary transversal deficiency had a dental Class III relationship, a -3 mm overjet, and a 3 mm overbite. It was determined that the patient was in the mp3 = era after radiographic analysis of her hand-wrist image. The patient with the skeletal class III anomalies (ANB= -3°) characterized by maxillary retrognathia (SNA=76°) had to be treated with a bonded RME and face mask appliance. The rapid expansion protocol required that the screw be twisted twice daily. The face mask appliance was applied when the suture was opened. A force of 400 g is applied to one side of the elastics to place them at a 30° angle with the occlusal plane. After using the face mask appliance for six months, Class III skeletal and dental anomalies, including anterior crossbite, were successfully treated, and successful improvements in the soft tissues were achieved.
Results: At the conclusion of using the rapid maxillary expansion with face mask, the patient's ANB value, which was -3° at the start of the treatment, was found to be +3°.
Conclusion: At the end of the treatment, the patient's aesthetic profile was acceptable. Ideal overjet and overbite relationships as well as class I molar relationships were achieved.
