Alt Tek Keser Çekimli Hastanın Şeffaf Plak İle Tedavisi

Özet

Amaç: Günümüzde estetik beklentilerin artması sonucu ortodontik tedavi ihtiyacı olan bireyler daha az görünen ortodontik apareyleri tercih etmektedirler. Bu nedenle konvansiyonel yöntemler ile uygulanabilen sabit ortodontik tedavilere alternatif olarak farklı estetik tedavi seçenekleri geliştirilmiştir. Bu olguda moderate çapraşıklığı olan hastanın şeffaf plak ile tedavisinin sunumu planlanmaktadır.

Olgu Sunumu: Estetik ve fonksiyonel şikayetlerle başvuran 34 yaşındaki iskeletsel ve dental Sınıf I maloklüzyona sahip erkek hastanın klinik muayenesinde üst orta hatta 1 mm sağa sapma, alt orta hatta 2 mm sola sapma, 3 mm overjet ve 2 mm overbite saptanmıştır. Aynı zamanda 12 numaralı dişte çapraz kapanış ve 33 numaralı dişte bukkal nonoklüzyon saptanmıştır.

Hastanın taraması iTero Element 2 (Align Technology) cihazı ile yapılarak ağız içi mevcut durumu dijital olarak bilgisayar ortamına aktarılmıştır. ClinCheck tedavi planı oluşturulmak üzere reçeteler oluşturulup Invisalign Technology firmasına gönderilmiştir. Alt çenesinde 5 mm yer darlığı tespit edilen hastanın ClinCheck tedavi planlaması sonucu alt tek keser çekimine karar verimiştir. Invisalign şeffaf plaklar (Align Technology Inc, Santa Clara, Calif) geldikten sonra ataşman plağının uyumu kontrol edilmiş ve ataşmanlar yapıştırılmıştır. Hastaya 7 gün geçtikten sonra bir sonraki plağa geçmeleri söylenmiştir. Tedavi sonu retansiyon protokolü için hastanın alt ve üst çene sabit retainer yapılarak tedavileri bitirilmiştir.

Sonuç: Hastanın tedavisi 18 ayda tamamlanmıştır. Hasta tedavi sonunda kabul edilebilir estetik bir profile sahip oldu. Angle sınıf I ilişki ve ideal overjet, overbite elde edildi. Şeffaf plak tedavisi ile hastanın daha iyi estetik, konfor ve ağız hijyeni sağlanmıştır.

Aim: Today, as a result of the increase in aesthetic expectations, individuals who need orthodontic treatment prefer less visible orthodontic appliances. Therefore, different aesthetic treatment options have been developed as an alternative to fixed orthodontic treatments that can be applied with conventional methods. In this case, it is planned to present the treatment of a patient with moderate crowding with clear aligners.

Case Report: In the clinical examination of a 34 year old male patient with skeletal and dental Class I malocclusion who presented with aesthetic and functional complaints, 1 mm right deviation in the upper midline, 2 mm left deviation in the lower midline, 3 mm overjet and 2 mm overbite were found. At the same time, crossbite was found in tooth number 12 and buccal nonocclusion was found in tooth number 33.

The patient was scanned with the iTero Element 2 (Align Technology) device and the current intraoral status was digitally computerized. As a result of the ClinCheck treatment planning of the patient, who was found to have 5 mm crowding in the lower jaw, it was decided to extract the lower single incisor. After the Invisalign clear aligners (Align Technology Inc, Santa Clara, Calif) arrived, the fit of the attachment plate was checked and the attachments were bonded. The patient was instructed to move on to the next aligner after 7 days. For the end of treatment retention protocol, the patient’s lower and upper jaw fixed retainers were applied.

Conclusion: The patient’s treatment was completed in 18 months. At the conclusion of the treatment, the patient aesthetic profiles were acceptable. The ideal overjet, overbite, and angle class I relationship were achieved. The clear aligner treatment improved the patient oral hygiene, comfort, and appearance.
