Background: Endobronchial aspergilloma is a rare disease entity of pulmonary aspergillosis and occur usually in the immunocompetent patient. In the present study described endobronchial aspergilloma which appears to be mass in a patient with B-cell acute lymphoblastic leukemia. Case Report: A 27 year old patient diagnosed with relapsed B-cell Acute Lymphoblastic Leukemia was consulted to our pulmonary service with progressive dyspnea, cough, fever and an abnormal chest x-ray. The complete blood count results were normal at admittance to the hospital and were reported as white blood cell 500/mm3, (neutrophils 41,7%, lymphocytes 49.0 %, and eosinophils 0.1 %), hemoglobin 8.5 g/dL and platelets 18000/mm3 after chemotherapy. The CRP level was 257mg/I. Computed tomography revealed multiple consolidated areas distributed randomly in the lung. In the diagnostic flexible bronchoscopy, distal the right main bronchus and the right lung middle lobe bronchial entrances were narrowed by a mass (Figure 1). A biopsy was obtained from the white mass at the entrance of the middle lobe bronchus. The histochemical evaluation revealed fungal hyphae that stained with PAS and GMS and was consistent with Aspergillosis. Voriconazole was started. Conclusion: Finally, although Endobronchial aspergilloma are rare, it should also be considered in the differential diagnosis of endobronchial masses.
