Amaç: Bu olgu sunumunda amaç, unilateral posterior çapraz kapanışa sahip adolesan dönemdeki erkek hastada, Mc Namara tipi hızlı maksiller ekspansiyon uygulaması sonuçlarını sunmaktır.
Materyal-Metot: Kronolojik yaşı 16 yıl olan erkek hastanın ağız dışı muayenesinde yüz hatlarının orantılı ve simetrik olduğu gözlenmiştir. Hastanın ağız içi muayenesinde Angle Sınıf 2 subdivizyon molar ilişki, tek taraflı posterior çapraz kapanış, 2 mm overjet, 1 mm overbite, üst orta hattın 2 mm solda ve alt orta hattın da 2 mm sağda olduğu ve sağ tarafta tek taraflı çapraz kapanışa sahip olduğu belirlenmiştir. Model analizinde maksiller arkta 1 mm yer darlığı, mandibular arkta ise 5 mm yer fazlalığı vardır. Sefalometrik analizde iskeletsel Sınıf 1 ilişki ile beraber normodiverjan dik yön büyüme modeli gözlenmektedir. Posteroanterior sefalometrik radyografide maksiller genişliğin azaldığı belirlenmiştir. Bu sebeple de bu hastada sol tarafın sabit tutulması ve sadece sağ taraftaki çapraz kapanışın çözülmesi adına macnamara apareyinin sol tarafına akrilik blok eklenerek ekspansiyon protokolüne başlanılmıştır. Ekspansiyon protokolü günde 2 tur olarak belirlenmiştir. 10 tur sonunda midpalatal suturun açıldığı klinik ve radyografik olarak gözlenmiştir. Toplam 36 turdan sonra retansiyon tedavisine geçilmiştir.
Bulgular: Hastamızın total tedavi süresi retansiyon fazı ile birlikte 7 ay sürmüştür ve hızlı maksiller ekspansiyon apareyi kullanımı ile transversal ilişki düzelmiştir.
Sonuç: Hastaya yapılan tedavi sonucunda kabul edilebilir bir estetik görünüm elde edilmiştir.
Objective: The purpose of this case report is to present the results of a Mc Namara type rapid maxillary expansion in an adolescent male patient with a unilateral posterior crossbite.
Materials and Methods: The 16-year-old male patient's extraoral examination revealed proportional and symmetrical facial contours. The intraoral examination revealed an angle class 2 subdivision molar relationship, unilateral posterior crossbite, 2 mm overjet, 1 mm overjet, upper midline 2 mm to the left and lower midline 2 mm to the right, with a unilateral crossbite on the right side. The casts analysis showed a space deficiency of 1 mm in the maxillary arch and a space excess of 5 mm in the mandibular arch. Cephalometric analysis revealed a class 1 skeletal relationship with a normodivergent vertical growth pattern. Posteroanterior cephalometric radiographs revealed a reduced maxillary width. Therefore, in this patient, an acrylic block was added to the left side of the Mc Namara appliance to stabilize the left side and resolve only the right-sided crossbite, initiating the expansion protocol. The expansion protocol was set at 2 rotations per day. After 10 rotations, the opening of the midpalatal suture was observed clinically and radiographically. After a total of 36 rotations, the patient was converted to retention treatment.
Results: The total treatment time for our patient, including the retention phase, was 7 months, and the transversal relationship was corrected with the use of the rapid maxillary expansion appliance.
Conclusion: The treatment resulted in an acceptable esthetic outcome for the patient.
