Özet

Ejaculatory duct obstruction (EDO) represents a surgically correctable cause of male infertility, accounting for approximately 1–5% of infertile men. The transurethral resection of the ejaculatory ducts (TURED) remains the standard therapeutic procedure for both diagnosis and treatment. This chapter provides a comprehensive overview of the pathophysiology, diagnostic evaluation, surgical techniques, and potential complications associated with TURED. Special emphasis is given to patient selection criteria, the role of transrectal ultrasonography, and emerging approaches such as TRUS-guided methylene blue instillation and endoscopic seminal vesiculoscopy with holmium laser. Common complications—including ejaculatory dysfunction, persistent obstruction, hematuria, pelvic pain, incontinence, erectile dysfunction, and rectal perforation—are discussed in terms of pathogenesis, prevention, and evidence-based management algorithms. Current data indicate that successful outcomes depend on accurate localization of the ductal orifices, preservation of the verumontanum, and meticulous surgical technique. Through structured prevention and management strategies, TURED can significantly improve fertility outcomes and patient quality of life while minimizing morbidity.
